Being a Cleric for the last 15 and counting years I would have to say my favorite clickie from long ago is Water Sprinkler of Nem Ankh. Up until they gave us the faster casting AA 96% Rez it was my go to clicky for MANY raids, groups and corpse runs lol.
 
It has to be the Pre-Nerf Circlet of Shadows There were so few on the server and owning one was a status symbol for years and years after Kunark was out. Instant cast invis on the run or if you were good enough you could Feign Death and spam click it while standing up to invis before the mobs could re-aggro. It was a puller's wet dream.
 
On my ranger my favorite clicky was always the Lizardscale Plated Girdle. The best part was probably how I camped it. I set an audio trigger and stood her spawn point semi-afk. A couple other people showed up claiming that the mob was "contested" so even though I had been there for hours they were going to kill it when she spawned. Eventually they AFK'd so I flustered bolted a mob behind them so it would roam back and hit them from behind. Luck would have it that right as the Cragbeast Queen spawned the one little mob I had made attack them FINALLY got them to purple (This took like three hours!). When she spawned their audio trigger went off and they returned to the game to watch me kill the Cragbeast Queen while they couldn't move because they were purpled.

to explain : you can have instant illu and no recast time for the fabled earth illusion each click (normaly 1800 sec for this buff ) so a snare proc + 5% rune each click unlimited :)
for fun too http://everquest.allakhazam.com/db/item.html?item=106473 it s temporary but in illusion ring will never fade ... so illusion gnome for all class in 5 min quest !! ( tp to chapterhouse , invu ,reclaim quest and have robes and leave quest :)
 
Clickies have Always been a big part of my main. Hard to pick 1 as the top.

Currently I may have to vote for Bulwark of Many Portals (3 Charges) which is summoned by my Mage's favorite click, Cauldron of Countless Goods.

This has been an awesome insta gate item for the ages... Saving many a wipe. ISBoxer: Broadcast Key, rotate main bar (all toons bars setup the same for some stuff) Click link to Bulwark! Use when Wizzy Exodus is not an option.

Old School favorite Click - Amulet of Necropotence AKA Key to the City erm, Norrath.

oh I could have said staff of forbidden rites too, I use that pretty often for the good battle rez! (too bad people cry about a 90% rez as if exp is hard to get)
