I am not using RedGuides or I am using RedGuides, the same thing happens. It only happens when I am playing Everquest when this happens.

I am sitting there playing EQ. multiple toons, and one or more of my screens freeze. Then my fan speed on my GPU speeds up to maximum and then both monitors goes black. After a few mins, the montiors come back on and any sceen that had a Everquest screen running has turned white and "relistFailed" error occurs. If this was happening on anything else that I am doing on my PC, I would not be making this post.

Any suggestions would be greatly appreciated.

Hey medic, hope you're well bro.

"relistFailed" and not "ResetDevice Failed"? can you just double check and confirm that.

Also:
Not using fullscreen mode
Not using mixed eq resolutions
Not using diff eq resolutions from desktop

Doing ok. Just had my SI Joint Nerves burned at a ripe old temperature of 800 degrees Farenheit. Other than that doing ok. How have you been doing?

Definitely relistFailed

Also:
EQ is in Windowed Mode
EQ resolution is 3840 x 2160
Desktop resolution is also 3840 x 2160 60hz

I don't know how to change the refresh rate in game

FYI my GPU is the GeForce RTX 5090

As I said before, I was not and have not been using RedGuides or Refetch ...mainly because I got back into playing again just after a major update. So, Goodle AI says the following "
AI Overview

An error message containing relistFailed typically points to a software or graphics pipeline routine failing to re-initialize, list, or reset a display surface or swapchain (often related to custom game engines, emulators, or specific third-party graphics hooks).

Common Causes
    • Overlay and Hook Conflicts: Background utilities like RivaTuner Statistics Server (RTSS) or MSI Afterburner inject code into the graphics API pipeline. When an application attempts to reset or re-query display lists/swapchains, these hooks can block the process and cause a relistFailed or similar initialization error. [1]
    • Window State and Resolution Changes: Changing display modes (such as toggling between fullscreen and borderless windowed mode, or altering resolutions while the graphics API tries to refresh the surface) can cause the context reset to fail.
    • Corrupted Shader or API Cache: Outdated or corrupted local DirectX/Vulkan cache files can cause the application's graphics calls to crash when trying to re-enumerate device displays or pipelines. [1]
    • Outdated Graphics Drivers: Bugs in the display driver can prevent proper handling of context recreation or surface resets during heavy graphical loads. [1]

Troubleshooting Steps
    • Disable Overlays: Turn off background recording or monitoring software like Discord overlay, GeForce Experience, or RTSS. [1]
    • Clear Graphics Caches: Delete temporary shader cache folders in your local app data directories or clear them via Windows Disk Cleanup. [1]
    • Clean Install Drivers: Use Display Driver Uninstaller in Safe Mode to completely wipe old graphic drivers before installing the latest version from your manufacturer. [1, 2]
    • Adjust Display Settings: Launch the application in windowed or borderless mode rather than exclusive fullscreen via configuration files if the app crashes on startup.

If you can share the name of the game or software throwing this error and your GPU model, I can give you more precise steps to fix it.

well i hadn't heard of relistFailed error before - but i'd start with the usual

- stuff i previously mentioned which you checked
- i'd check windows updates
- i'd check vid driver updates
- i'd check doing an eq file validation check via the eq launcher

If nothing that Sic recommended works, try renaming your EQ install folder to something like "EQ-old", then run the launcher and allow it to do a complete new install. I had something similar happen to me a month or so ago and ended up having to do this myself.

Regarding video drivers: If windows update updated your video drivers, and you were previously using a driver you downloaded directly from Nvidia (or AMD, seen this with both), it can cause problems. I would recommend downloading the most recent driver directly from the OEM and doing a clean install when prompted. I would also select to install only the drivers and not any of the extras, as those can cause problems themselves.

Lastly, based on the description of your problem, there could potentially be a heat or VRAM issue with your video card, though this is less likely than the other potential causes, assuming that its been working normally in the past. You could try removing it and running EQ just using the integrated video on your CPU. Even those are enough to run a single EQ client at 4k for testing purposes.
