Hey guys, wanted to give you a followup with more info/details...
First of all, I think your pushes fixed the situation. I'm having a hard time reproducing those deaths and it seems such a rare situation that you probably shouldnt worry about it!
I have a few theories on what could have happened, maybe you can help me clear it...
First I thought it could be terrain, the situation happens in Feerott in a non flat terrain area, I thought it could be some Z-axis issues since I've never seen those deaths in my other group hunting in a more flat area. But that doesnt seem very likely...
Secondly and that's probably my best unfounded guess, I think it might be linked to my computer performance. When i had those deaths often i was running 12xEverquest and 1xpathofexile2 (kinda perf hungry recent game) on my computer, stuff was kinda laggy but it seemed to hold fine... I also only ever experience those puller deaths when that specific EQ window was in the background. If I was watching, everything was fine. Now that I've stopped running poe2 alltogether, I seem to never have those deaths anymore. Could it be that? not too sure
And lastly I thought about maybe the puller's ability, it happened on my war using taunt ability as pull, also melee attack as pull and also on my monk using distant strike ability as pull but never happens on my SK using spear of blablabla spell as pull.
I have also watched a very rare phenomenom (only once) where the puller target (a roamer) would instantly and magically warp a long distance away right as it's pulled. It looks kinda like a client/server mismatch of some sort. I saw it happen live on the map view opened. Right as the mob gets aggroed, it "teleported" way out there, like range 2000 I would say, way out of the pulling radius. It was not a gate spellcaster or anything, it felt more like a warp client/server thing or whatever. So the mobs remains on aggrolist and starts running toward my puller who was also running toward the new far away mob location, resulting in a situation where puller dies out there. I did not have debug verbosity ON at the time. Hard to provide more details here
But yeah as I said earlier, consider this a non issue, I've had long unattended sessions going ultra fine since then. Dont waste your time on this

I'll keep reporting other issues as I encounter them, I've noticed some minor stuff like my berserker trying to spam some steel frenzy ability (requires 90% or less HP) while he's at full life but I'll make separate posts about those to keep things organized!
keep up the good work!
