I saw this on kiss ini and wasnt sure how people use it or what they put in there, does anyone use this section?

Yeah it's eqmule's ka mod, a split from KA10, which is inlcuded in Vanilla. Those are high priority things you'd want KA to handle. Instaheals or clicky items. Term OhShit is from mq2melee conditional triggers called holyshit (do this immediately regardless of what is going on) and downshit (do this only if I'm out of combat.)
