I had a problem with flying kick as well. It would flying kick some of the time, but would frequently just do it once in the fight and never again. I only briefly played a monk so I forgot all about it. My easy fix was to make a holyshit. It made flying kick go off every time it was up...

I am also seeing the pulling part act a little funky on occasion. I was testing it in the bat room in forgotten halls. Sometimes there would be bats flying all around my toon and it would go into the 30 sec wait once or twice. It would spot the very same bats after that and go to town on...
