When I start MQ then the game I see in character select the traditional MQ boxes with news, indicating to me that MQ is working. The second I load into the game or zone these boxes go away. I have to manually refresh injections to get it to work. Is there a way I can get refresh injections to occur automatically when I zone? or have them not unload when i zone? is there a configuration I can set somewhere?

If MQ2 is unloading itself every single time you cross a zone line, you have something very wrong. However, with such little to go on, it's gonna be a crap shoot trying to guess what that is.
I would start with a list of active plugins that you have running by default. Maybe pare that down to only absolutely essential plugins and then start adding things back one by one until you find the culprit. Just because it comes with the VV package doesn't mean it should always be loaded or that it doesn't conflict with something else.

Sounds like you're trying to use it on a truebox server. Someone asked what server you were on and you said "live" which is not a server.

As others have indicated this is the normal behavior when you are operating on a Truebox server, i.e. a server that expressly prohibits any kind of automation. MQ will attempt to unload itself to keep you out of trouble (maybe you forgot you were logging into a Truebox server for example...) but if you keep re-injecting it, you're just asking for problems.
