I have been playing with this for days to see what was happening. I have my tab key configured for cycle nearest NPC. At times when it is stuck, I hit tab to find a mob on that toon several times and it suddenly starts pulling for a while, however, right clicking mobs in the map window never worked.
LoS when using NAV is causing it to fail and return home. When that happens, it stops.
