Imo, adding that gem slot was a "low hanging fruit" for reason to get the expansion (ToB), almost no classes "needed" an extra slot - some classes don't even have anything extra to use in their rotation, and imo, was lazy of DBG
We do use it if a spell is in it - byos or not-byos/byos-custom - and since we don't memorize anything there, we're also not going to remove anything *you* place there.
As I mentioned, over the past 9+ months I've talked about it a few times, and until otherwise, classes that we don't slap something in there will have it empty, this isn't "lazy" or "dont care" it is intentional.
I suspect we're going to get another entire full spell line in the new future for all spell classes and don't want to get pigeon holed into juggling shit.
I have spent quite a bit of time going over what it might look like to either: adjust the misc gem to that - or allow a user to specify the misc gem, and as of now have decided not to change that.
A.k.a. because we don't put something there
