Ok so this is weird. I left my chars logged in afk in guild hall like i usually do while I was asleep. I had been playing normally before that, nothing new.
Went to play when I woke up and any /casting would only work the first time, once per zone. I seem to recall someone else posted this exact same thing but I can't find it now. I didn't do anything different between the time it started working and when it stopped. Same macros, same plugins, no changes at all.
I turned on /castdebug and it's something to do with mq2cast not knowing it's finished casting the first spell.
I'm still working on different code fixes to make it work again, but I don't know why it stopped working in the first place. If you had this happen and got it working let me know what you did. I've also (now) tried recompiling everything and even used the vanilla compile, can't seem to get it to work. Open to suggestions if you've seen this before. I'll poke at it again after a bit more sleep, hopefully I'm just overlooking something.
